We are proud to offer our community a new preventative health program, HeartAwareTM. We invite you to complete a confidential seven-minute online risk assessment to determine if you are at risk for heart disease. Upon completion of the assessment, you will receive a complete report that you can print and share with your primary care physician which outlines your levels of risk.
You will be asked, although not required (can respond "unsure"):
- Your blood pressure
- Your cholesterol levels, including HDL & LDL
Should you have risk factors identified, participants will be invited to come to LiveWELL Carolinas!, our community resource center at Northlake Mall, and receive a FREE screening, including a blood pressure and cholesterol check. Those who qualify will be referred for further testing to help prevent heart disease or heart attacks and to reduce your risk factors for heart disease with early intervention.
